On 04-Feb-2026, the patient was vaccinated with Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine, Live (M-M-R II) (0.5 ml, total; strength, route, lot# and expiration date were not reported) for prophylaxis at a pharmacy. The vaccine was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT ) (lot# and expiration date were not reported) and left out for 40 minutes before being administered (Product storage error, Product preparation issue).
