Symptom narrative

Patient presents to office with mom for concerns of lump/ bump on left arm. Patient states she has been having pain in that area since end of December after receiving 2nd bexsero and 3rd HPV on 12/31. Pain stopped after a week of receiving vaccines. The pain started again 4 days ago. Feels hard bump on vaccine site. Patient is able to move shoulder but has pain at injection site with certain movement. Denies numbness, tingling or pain radiation. Had some common cold symptoms around 1st week of January. Denies current fever, chills, cough or congestion.

Cyproheptadine 4mg every night, dapsone 7.5% topical gel, Hydrocortisone 2.5% lotion, mefenamic acid 25o0mg (2 tablets at onset of pain and 1 tablet every 6 hours as needed afterwards), Retin-A 0.06% topical gel, Winlevi 1% topical Cream
