Patient stated on her electronic consent form that she was not sick and did not answer the question if she have any open sores/nothing mentioned to tech or pharmacist pre-vaccination. She went to pharmacy [name withheld] to get the shingles vaccine and the associates there sent her to us. [name withheld] did not get told till after the vaccine was administered that she has had shingles for a month with current open sores. Then we learned from pt Active Shingles "a month ago"-per pt, with open, visible bloody sores (4 sites)-per technician [name withheld] rough observation. RPh [name withheld] checked first part of vaccine, then [withheld name] CPhT went to administer vaccine, both without the knowledge because the patient did not say her shingles diagnosis or sores were of concern. Arm injection site was not area of sores. elsewhere looked like picked scabs so would not have known
