Symptom narrative

Patient came to pharmacy for an RSV and prevnar 20 vaccine appt. Pt was accidently administered prevnar 20 and shingrix instead of arexvy and prevnar 20. Both arevxy and shingrix are made by GSK and have similar packaging and both need to be reconstituted in the same manner. Pharmacist took wrong vaccine out of fridge. Pt called 2 days later because her paperwork said shingrex instead of arexvy. She reported having felt tired and achey after vaccine administration and that her arm was very sore.
