Patient seen in office by Dr on 2/13/26. Patient had 10cm red ring surrounding injection site. Ring has been growing since injection reaction was noticed on 2/12/26. Thigh also shows swelling but not hurting patient. Patient started itching site on 2/12/26. Treatment plan listed below by Recommend oral Benadryl q6 hours scheduled for the next 24-48 hours, then daily PRN - Ordered Mupirocin 2% TID x 3-5 days as well given central excoriation - Ok to continue children's Motrin/Tylenol - She is aware to seek urgent attention for high fever >103F, dyspnea, vomiting/diarrhea, or any other concerns; otherwise, will see her Tuesday 2/17/26 for a recheck (or Monday for nurse visit if needed) - Mom is going to remark edges of erythema at home (provider could not find marking pen in office- Mom then offered to mark once they get home)
