Symptom narrative

Patient was 2 months old at the time of the appointment and was supposed to be given PCV 15 and without consent was given the incorrect vaccine (Capvaxive) by mistake. This mistake went unfound until 1/6/25 almost 3 months later after because another incident on 12/18 where an extra dose of Hep B was given to patient by mistake that prompted a review of her vaccine records.
