Pt. was mistakenly given a TDAP/Boostrix vaccine instead of Men B/ Bexsero. Wrong vaccine was picked from the fridge and gave it. Charted after giving the vaccine and it's when she realized she gave the wrong vaccine. She immediately informed the provider and nurse manager. Provider informed the parent of what happened. Corrective action reviewed with the nurse to always verify the 5 RIGHTS of medication and vaccine administration. (Right patient, right medication, right dose, right route and right time. Also to make sure to document in the chart the lot number in the EMR system before administering the vaccine. This helps to make sure the lot# matches the vaccine your giving.
