Reconstitution of Hiberix with Liquid Diluent not Supplied by GSK; vaccine was administered to a patient with sterile water; This non-serious case was reported by a nurse via call center representative and described the occurrence of wrong solution used in drug reconstitution in a 1-year-old male patient who received Hib (Hiberix) (batch number 95DT7, expiry date 13-JUL-2026) for prophylaxis. On 10-FEB-2026, the patient received Hiberix. On 10-FEB-2026, an unknown time after receiving Hiberix, the patient experienced wrong solution used in drug reconstitution (Verbatim: Reconstitution of Hiberix with Liquid Diluent not Supplied by GSK) and inappropriate dose of vaccine administered (Verbatim: vaccine was administered to a patient with sterile water). The outcome of the wrong solution used in drug reconstitution and inappropriate dose of vaccine administered were not applicable.
