Only diluent administered; Inappropriate dose of vaccine administered; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a female patient who received Herpes zoster (Shingrix) (batch number GC4X3, expiry date 17-JUN-2027) for prophylaxis. On 09-FEB-2026, the patient received Shingrix. On 09-FEB-2026, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: Only diluent administered) and inappropriate dose of vaccine administered (Verbatim: Inappropriate dose of vaccine administered). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK receipt date: 11-FEB-2026 The patient mentioned that she was still feeling all the effects as if she got the vaccine, but the patient does not actually get the actual solution that will provide her the immunity.
