The occurrence of additional non-serious events is detailed below. The patient's past medical history included Hysterectomy. Concurrent medical conditions included High cholesterol (has high cholesterol but cannot take statins). On 26-Apr-2021, the patient received first dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular use) 1 dosage form. On 04-Jun-2021, received second dose of mRNA-1273 (Moderna COVID-19 Vaccine) (Intramuscular use) dosage was changed to 1 dosage form. On 04-Jun-2021, after starting mRNA-1273 (Moderna COVID-19 Vaccine), the patient experienced INAPPROPRIATE SCHEDULE OF PRODUCT ADMINISTRATION (First dose of Moderna COVID vaccine on 26-Apr-2021 and the second dose on 04-Jun-2021). In 2021, the patient experienced LOSS OF CONSCIOUSNESS (blacked out / was watching tv and everything went double, she stood up and blacked out and went straight down/blacked out 6 times so far) (seriousness criterion medically significant), HOT FLUSH (hot flashes shortly after the first blackout in the end of 2021), BRAIN FOG (experiences a foggy brain that started after the first time she blacked out), HEADACHE (bad headaches) and VOMITING (vomits after blacking out). In February 2022, the patient experienced VISION BLURRED (could not see out of her right eye, everything was a blur / can't see really good / has only one good eye). In 2023, the patient experienced BURNING SENSATION (troubles with legs and feet experiencing a burning sensation and stated they ache like a toothache). On 16-Jun-2023, the patient experienced PERIPHERAL SWELLING (both of her legs swelled up like a balloon). In April 2025, the patient experienced HIP FRACTURE (fell to the floor and broke her hip and required surgery) (seriousness criterion medically significant) and FALL (fell to the floor and broke her hip and required surgery). In 2025, the patient experienced PRURITUS (everything itches real bad), GAIT DISTURBANCE (cannot walk, loses balance, and has to use walker or cane for assistance), POLLAKIURIA (started going to the bathroom every 1 and a half hours, throughout the day and night to void her bladder), AGEUSIA (has no taste for food) and BALANCE DISORDER (cannot walk, loses balance, and has to use walker or cane for assistance). In November 2025, the patient experienced RHINORRHOEA (she gets these cold chills, her nose starts to run, and she has to go to bed to get warmed up) and CHILLS (she gets these cold chills, her nose starts to run, and she has to go to bed to get warmed up). Her hysterectomy incision just had big pus-filled sores.). On an unknown date, the patient experienced NERVOUSNESS (nerves are shot) and POST-ACUTE COVID-19 SYNDROME (long-term COVID). The patient was treated with Surgery (Surgery) for Hip fracture. No concomitant medication was reported. Patient has no vaccines within the past 4 weeks. It was stated she received the Moderna COVID shot in 2021 and had nothing but side effects since then. It was stated that she blacked out for the first time in October or November 2021. It was stated that she was watching tv and everything went double, she stood up and blacked out and went straight down. It was stated that she also vomited after blacking out. She had blacked out 6 times so far and stated that nothing specific brought it on, and there were no warning signs. It was stated that her last black out was April 2025, she was in the basement doing laundry, she blacked out, fall to the floor and broken her hip and required surgery. It was stated that she experienced a foggy brain that started after the first time she blacked out. She started getting bad headaches and hot flashes shortly after the first blackout in the end of 2021. She received three shots in her eye and stopped as it was not doing any good. Patient had a stent put in her leg 15-Jun-2023. After the stent, the next day, she had both of her legs swelled up like a balloon and the doctor did not know what to do with her. It was stated that the swelling has gone down, but she had been having troubles with legs and feet experiencing a burning sensation and that they ache like a toothache. It was also stated that for 6 months ago, everything itches real bad. It was reported 1 and a half months ago, she started getting sores inside and outside of her mouth, up her nose, across her stomach, in her private areas, along her hysterectomy incision, up her butt crack. Her hysterectomy incision just had big pus filled sores. It was stated that the sores move around, and she never knew where they were going to be. She had medicine from the doctor that provided some relief, but they came back. It was stated that patient could not see really good and she had only one good eye. She got these cold chills, her nose started to run, and she had to go to bed to get warmed up. it would go away and might come back during the day which started in November 2025. It was stated that her main problem, which started over a year ago, was that she could not walk, she lost balance and had to use walker or cane for assistance. Patient doctor put her on 3 different medicines, and she took 2 of them and on Saturday, she took one more and she woke up the next morning and her right eye hurt real bad. Patient eye had bad pains all the way around it. She went to the eye doctor and was diagnosed with a stroke in her eye diagnosed December 2025. She could not take any medicines or vitamins, so she got sick and got the shakes from them. It was stated that her nerves were shot. It was stated that there was something new every day. It was also stated that about 7 months ago, she started going to the bathroom every 1 and a half hours, throughout the day and night to void her bladder. She also had no taste for food, which started about a year ago. She had tried the Clinic, and she had long-term COVID. There was nothing they could do for her. She had one doctor tell her it was all in her head, but she knew it was not. She had seen her new HCP once, so did not know a lot. It was unknown if the patient experienced any additional symptoms/events. No specific treatment drugs were reported.
