The patient's medical history was not reported. The patient's concurrent conditions were not reported. Concomitant therapies were not reported. On 30-Dec-2025, the patient was vaccinated with Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX), Powder and solvent for solution for injection, (lot #Z008197, expiration date: 30-Apr-2027) 0.5 ml, total as prophylaxis. The vaccine was reconstituted with sterile diluent (Hollister-Stier STERILE DILUENT), Solution for injection (lot # and expiration date were not reported). During the administration of the vaccine, the patient moved resulting in only half of the dose being administered (Underdose). The nurse then discarded the remaining vaccine. No effects reported.
