No adverse effects were reported; GARDASIL 9 was confused with PREVNAR 20; GARDASIL 9 was confused with PREVNAR 20 and inadvertently administered to a 60 year old female patient today, 2/13/2026; This spontaneous report was received from a nurse (registered nurse) and refers to a 60-year-old female patient. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 13-Feb-2026, Human Papillomavirus 9-valent Vaccine, Recombinant (GARDASIL 9) Suspension for injection (valid lot #Z011090, expiration date: 07-Aug-2027) at dose 0.5 mL was confused with Pneumococcal vaccine conj 20v (CRM197) (PREVNAR 20) and inadvertently administered to a 60-year-old patient for prophylaxis (wrong product administered, product confusion). Nurse stated the wrong box was grabbed, and it was not noticed until after it was administered to the patient. No adverse effects were reported (no adverse event).
