After receiving Hep A, Cholera, Polio, Rabies #1, and Typhim, pt. had a vasovagal response at around 10:00am. Pt. reports no hx of fainting with vaccination. Pt. was recovered with ammonium and supported by vaccination chair. Pt. sustained no fall or head injury. Pt. remained diaphoretic and uneasy. Pt. then had a second vasovagal response. Pt. still sustained no fall or head injury. Upon recovery, pt. vomited. At this time, BP 98/65. Pt. insisted on needing to use the restroom, reporting abdominal pain. This RN assisted pt. to bathroom. Pt. had diarrhea and vomiting in the restroom. This RN called EMS at this time. Pt. AOx4 to orientation questions but unable to remember husband phone number and reports feeling "out of it" with "tunnel vision." EMS arrived within minutes. At this time, BP 92/68 with HR 60. EMS started fluids and brought pt. to medical clinic. This RN called husband (listed emergency contact) per pt. request and LVM. Pt. did not receive YF vaccine despite it being drawn up. Ticket submitted. Pt. will need YF vaccine for travel, and it has already been charged for this vaccine. VAERS and Medical Management forms complete. Nurse Hotline and Nurse Manager notified of incident.
