A patient (age and gender not provided) received pneumococcal 20-val conj vac (dipht CRM197 protein) (PREVNAR 20), as dose number unknown, single (Batch/Lot number: unknown) for immunisation. The patient's relevant medical history and concomitant medications were not reported. The following information was reported: DEVICE BREAKAGE (non-serious), outcome "unknown", described as "Dropped the Prevnar 20 and the tip broke / the tip broke when they were pulling it out of the refrigerator and it dropped". Additional information: It was reported that they dropped the Prevnar 20 and the tip broke. The reporter was calling to say that the vaccine was damaged the tip broke when they were pulling it out of the refrigerator and it dropped and she said they have taken out plenty out before and it has never broken like that.
