Symptom narrative

2 x syncopal episode post-vaccination ~2 minutes followed by another within a few minutes of regained consciousness from first episode. Light headedness reported and RN witnessed shaking and brief loss of consciousness (less than 20s each time) with a light head strike on wall behind exam room bed during 1st syncopal episode. MER called to assess patient, vitals checked and assessed by provider. Patient reports having not eaten much earlier in the day and slight headache pre and post vaccination.
