Symptom narrative

Site: Pain at Injection Site-Mild, Site: Swelling at Injection Site-Mild, Systemic: Chest Tightness / Heaviness / Pain-Mild, Systemic: Dizziness / Lightheadness-Mild, Systemic: Exhaustion / Lethargy-Mild, Systemic: Visual Changes/Disturbances-Mild, Additional Details: Patient stated 2 days after receiving the vaccine symptoms began to occur. Patient stated they had arm pain, blurry vision, tiredness, no appeitite and some chest pain. Patient was counseled on common side effects of the vaccinations and that they may last longer than others but normally begin within 24 hours of vaccination. Patient was recommended to go to the doctor office. MD called pharmacy to ask what vaccine was given and they prompted patient to go to urgent care. Patinet is ok.
