Symptom narrative

Site: Pain at Injection Site-Medium, Additional Details: Patient stated that he mostly feels the pain in both arms after receiving 3 separate vaccines the same day, I only see one vaccine administered but he was certain that he received 3 separate vaccines. He also mentioned that he has arthritis and was sure if the pain was from that but noticed that the pain got worst after receiving the vaccines on both arms.
