Symptom narrative

eleven years old, was due to have a Tdap but was given a DTaP in the form of Infanrix; This non-serious case was reported by a nurse via call center representative and described the occurrence of wrong vaccine administered in a 11-year-old patient who received DTPa (Infanrix) (batch number BH57H, expiry date 07-JUL-2026) for prophylaxis. Co-suspect products included Diphtheria vaccine toxoid, Pertussis vaccine acellular 5-component, Tetanus vaccine toxoid (Adacel) for prophylaxis. On 12-FEB-2026, the patient received Infanrix. The patient did not receive Adacel. On 12-FEB-2026, an unknown time after receiving Infanrix, the patient experienced wrong vaccine administered (Verbatim: eleven years old, was due to have a Tdap but was given a DTaP in the form of Infanrix).
