The suspect product included Pre-filled Syringe injection for COVID-19 prophylaxis and non-company product included Influenza vaccine for an unknown indication. On 05-Feb-2026, the patient received dose of mRNA-1273.251 (Spikevax LP.8.1 PFS) (Intramuscular use) .25 milliliter and dose of Influenza vaccine (unknown route) 1 dosage form. On an unknown date, the patient started Pre-filled Syringe (unknown route) at an unspecified dose. On 05-Feb-2026, the patient experienced VACCINATION SITE NODULE (the leg had a round bumped up area right around the vaccination site) and VACCINATION SITE ERYTHEMA (At the vaccination site, within 24 hours, patient had a red circle). On 06-Feb-2026, the patient experienced SKIN LESION (there were 6 lesions in mid upper chest. The leg had a round bumped up area right around the vaccination site, and it had been there since 05-Feb-2026. The rash was under the patient's chin by the folds of her neck. They looked viral when they first started out, it was raised, red, and there were 6 lesions in her mid-upper chest. Right under her chin, there were another 4 viral-looking lesions (a red spot with a little fluid-filled vesicle) and in the folds of her neck, there were another 5 lesions. It was unknown if the patient experienced any additional symptoms/events. Treatment medication was not reported.
