(name withheld) the vaccine coordinate for our health department, said this needed to be completed because patient received a 3rd dose of Hep B. The patient told the nurse during the hiring and on boarding she had blood drawn and was told to come and get another Hepatis B shot. The Patient told the nurse the incorrect information. The patient needed to return to another blood draw for Hepatis B
