Symptom narrative

During PCV20 attempted administration, patient moved. Neele entered the skin x 2 without medication administration. Administration completed with new PCV20 dose.

Current illness

Office visit on 1/6/2026 for petechiae rash around eyes and behind left ear, bilateral eye discharge, and rash on back (heat rash vs irritant dermatitis). Office visit on 1/13/2026 for follow up--rash and eye discharge resolved. Office visit for 12 mo WCC on 2/3/2026.

Trisomy 21; Congenital laryngomalacia; GERD (gastroesophageal reflux disease); Congenital nystagmus; and Other dysphagia
