Symptom narrative

Patient moved during Proquad immunization administration to right thigh. Staff had completed administration and in the process of activating safety. Mother let go of patient's hand and needle scratched his thigh. 3 superficial lacerations less than 0.1 cm in width. Laceratin length: 1st) 0.2cm, 2nd) 4cm 3rd) 2cm. No active bleeding, swelling. Bacitracin applied and band-aid placed. No other s/s.

Tonsillar and adenoid hypertrophy and Speech delay. Hx of tonsillectomy/adenoidectomy 2025.
