Symptom narrative

Received Two Doses of Heplisav-B Within Two Days; Initial report received on 20-Jan-2026. A pharmacist reported that a female patient inadvertently received two doses of HEPLISAV-B (hepatitis B vaccine) for hepatitis B immunization within two days. No medical history or concomitant medications were reported. On 14-Jan-1016, the patient received dose 1 of a hepatitis B vaccine [HEPLISAV-B (lot number, expiration date, NDC number, route, and site not available)]. On 16-Jan-1016, two days after receiving the first dose of HEPLISAV-B, the patient received dose 2 of a hepatitis B vaccine [HEPLISAV-B (lot number, expiration date, NDC number, route, and site not available)]. The patient came to a different pharmacy from dose 1 and inadvertently received a second dose of HEPLISAV-B. The patient received the second dose within two days from the first.
