Symptom narrative

I was seen 02/17/2026 at 12:45 PM by NP for an Annual exam. I was told by the medical assistant working with the said provider that I was due for the Hep A and Meningitis vaccine. I received both vaccines in the left arm. Both vaccines were injected into my shoulder. Pain, arm stiffness, loss of arm mobility, and left finger tingling started around 2:00 PM 02/17/2026. I contacted the after-hours on call nurse and relayed my symptoms and was told to go to the ED due to sweating after the immunizations. I was seen the next day by another provider within the Network for a second opinion and was diagnosed with SIRVA due to the vaccine administration error. I was prescribed Meloxicam and Prednisone to help with recovery. I was also given a referral for physical therapy. I reviewed my immunization records through the Patient Portal, and I noticed that only the Hep A vaccine was documented and not the Meningitis vaccines. I have since been in extreme amounts of pain, limiting my ability to work, and carry out my activities of daily living.
