This incident happened on 2/17/26. The client walked into the walk-in clinic while I was taking care of another client. I was assisting a client who brought in children and the atmosphere of the office was chaotic. The walk-in client wanted a pneumonia shot. I pulled a Tdap vaccine instead of the Prevnar 20 vaccine. I felt hurried and rushed and was stressed due to the amount of people in the office and I was distracted. I administered the Tdap vaccine and the client left. After the client left and the previous client with children left I was putting the vaccine into the record and I noticed I gave the wrong vaccine. I called my CNM and reported the incident. The next day, 2/18/26, I called the client and reported that I gave him a Tdap instead of a pneumonia shot. He thanked me for letting him know. The client was feeling fine and was not experiencing any adverse reaction to the vaccine. The client scheduled to get his pneumonia shot for the next week.
