No additional AE; Nurse reported that a patient received only 1/2 dose of MMR II due to a human error.; The nurse did not pull the full amount out of the reconstituted vial.; This spontaneous report was received from a Nurse (Registered Nurse [R.N]) and refers to a patient of unknown age and gender. The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 13-Feb-2026, the patient was vaccinated with Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine, Live (M-M-R II), Injection (valid lot #Z008973, expiration date: 16-May-2027), dose number 2, (1/2 dose) reconstituted with sterile diluent (DILUENT MRK) Solution for injection (lot # and expiration date were not reported) for prophylaxis. The nurse reported that a patient received only 1/2 dose of suspect vaccine due to a human error (accidental underdose). The nurse did not pull the full amount out of the reconstituted vial (product preparation error). No additional AE were reported (no adverse even).
