Symptom narrative

Immunization was ordered by ARNP for a 5 month old infant. ARNP left our practice on 1/23/2026. On 1/27/2026 I was covering the center and after reviewing the chart, realized the vaccine Kinrix is not appropriate for this age group. Patient did not have any reaction. I met with mom 2/17/2026 and discussed the event with her, and future recommendations, so he is immunized properly.
