Patient came into office and requested the Covid and Flu vaccines. He received the high dose flu vaccine, indicated by his age. However, he was given the wrong Covid vaccine. Patient received the pediatric dose of covid instead of the adult. Patient had no symptoms or reactions. He was notified of the provider's message to simply return for the full vaccine.
