The patient's medical history, concurrent conditions, and concomitant therapies were not reported. On 02-Feb-2026, the patient was vaccinated with an improperly stored Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD), Solution for injection, lot #Z014407 and expiration date: 10-Mar-2027 confirmed as valid, 0.5 mL administered by a healthcare professional once subcutaneously (site of administration not provided) for prophylaxis (product storage error). The suspect vaccine was reconstituted with sterile diluent solution for injection, lot #2171667 and expiration date 06-Feb-2028 confirmed as valid for pre-filled syringe. The administered suspect vaccine underwent a temperature excursion of 5 degrees C for 1368 hours. The reporter stated that the suspect vaccine was stored in the refrigerator upon receipt.
