Symptom narrative

Patient received both vaccines around 9:15 am in the clinic lab and then at 9:30 am, patient proceeded to have a venipuncture lab draw and upon drawing vial #4, patient became clammy and skin color change to white. Blood draw was stopped, Patient was given juice and vitals taken which were stabilize and patient no longer with syncope symptoms.
