Engerix-B second dose missing; Pediarix early third dose; This non-serious case was reported by a other health professional via call center representative and described the occurrence of drug dose administration interval too short in a 6-month-old female patient who received DTPa-HBV-IPV (Pediarix) (batch number TE499, expiry date 02-FEB-2027), (batch number Z9S4D, expiry date 04-JUN-2027) and (batch number Unknown) for prophylaxis. Co-suspect products included HBV (Engerix B) for prophylaxis. Previously administered products included Pediarix (received first dose on 29-SEP-2025), Pediarix (received second dose on 09-JAN-2026) and Engerix-B (received first dose on 27-JUL-2025 with batch number 3N2ND). On 09-FEB-2026, the patient received the 3rd dose of Pediarix. On an unknown date, the patient did not receive the 2nd dose of Engerix B. On 09-FEB-2026, an unknown time after receiving Pediarix and not applicable after receiving Engerix B, the patient experienced drug dose administration interval too short (Verbatim: Pediarix early third dose). On an unknown date, the patient had incomplete course of vaccination (Verbatim: Engerix-B second dose missing). The outcome of the drug dose administration interval too short and incomplete course of vaccination were not applicable. Additional Information: GSK Receipt Date: 16-FEB-2026 The reporter reported that patient received one dose of Hepatitis B vaccine on July 27th 2025 (unknown brand name, lot number provided pertains to Engerix-B). Then patient received 3 doses of Pediarix, dose 1 was given on 29-SEP-2025 , dose 2 given on 09-JAN-2026 and dose 3 given on 09-FEB-2026 . The reporter then highlighted that patient received third dose of Pediarix earlier than 8 weeks, but mentioned that patient reacted well. The batch number was not provided upon follow- up with the reporter.
