Symptom narrative

I went to get the vaccine, and my other Medical Assistant came to sign off with me. We both missed the Expiration date on the vile. I noticed the date after I had given the shot to the Patient. When I started to document the shot is when I had noticed the date. I then got my nurse LPN and we had both went to the Dr and office Manager. The Dr had advised me that there was no harm. I then went to the patient and his mother to let them know what had happened along with the LPN. I then had document on the patient's chart about the incident. The mom will come back in with patient next year to get the vaccine. Mom had scheduled appt.
