Symptom narrative

Member received the Anthrax and Flu Vaccine on 2/16/2026. On 2/17/2026, he noticed symptoms of a localized reaction (redness, warmth, and swelling) to the area of injection on upper Left arm. Patient reported that he has a history of having a localized reaction to all vaccines. Member arrived to clinic on 2/19/26 to receive another vaccine and reported this reaction. Deep red smaller then the palm of his hand at injection site noted. Measured reaction site with wheel at 4cm by 2.5cm and warm to the touch. Note has been added to his IMR record for future vaccines.
