12-22-2010 Patient rec'd varicella vaccine; invalid dose as had rec'd MMR vaccine on 11/29/2010 10-21-2025: Patient seen by provider & cleared to receive live vaccines due to hx Kawasaki dz. Provider also started patient on oral birth control pills (Sprintec). See question 9 above 01-20-2026 3:45 pm Patient received vaccinations - MCV4 and varicella 01-21-2026 11:00 am Patient reported to mother c/o's 12-22-2010 Patient rec'd varicella vaccine = invalid dose as she had rec'd MMR vaccine on 11-29-2010 (did not meet minimum interval between vaccines). 10-21-2025 Patient seen by provider & cleared to receive live vaccines due to hx Kawasaki dz. Provider also started patient on oral birth control - Sprintec. (See question 9 above for additional details). 01-20-2026 3:45 pm Patient received vaccinations - MCV4 and varicella 01-21-2026 11:00 am Patient reported to mother c/o's "head pain, fever, weakness, decreased appetite, and swelling above her eye". Mother states she used benadryl, which brought down the swelling. Adolescent continued to c/o of headache for 1 week & used excedrin for pain, helping some. Mother reports that 2 and 1/2 weeks after vaccination, patient developed "dry eyelids". Adolescent continues to c/o dry eyelids as of today 02/17/2026, despite the use of a moisturizer.
