Symptom narrative

Patient received third dose of Shingles on 12/13/2025. patient was not aware he completed series after receiving two doses from pharmacy (11/22/2020 & 09/26/2023). Patient returned to pharmacy on 02/18/2026 for "second dose" where error was discovered. He should not have received extra dose of Shingrix. Patient was not administered an extra dose (would have been fourth) past 12/13/2025. Patient was counseled and understood not to receive extra doses and he understood he had completed series. After a series of questions, patient confirmed he was unharmed and had experience no secondary side effects from 3rd vaccination.
