Patient presented to clinic for appointment with their PCP. During the visit, meningococcal ACWY vaccine (Menveo) was administered in error. Patient was supposed to receive meningococcal B vaccine (Bexsero). After administration, provider attempted to document the vaccine in the EMR and noted that the barcode would not scan. Provider then approached me for assistance. Upon review of the vial and Epic order, it was identified that meningococcal ACWY vaccine (Menveo) had been administered instead of the ordered meningococcal B vaccine (Bexsero). At the time the error was identified, the patient had already left the exam room. Provider was notified of the situation. The patient had no immediate adverse reaction at the time of administration and tolerated the vaccine without complication.
