Symptom narrative

Patient was given an extra does of the Hep A vaccine due to EHR system not populating vaccines that were previously given in office. Hep A dose was given based off of previous records from out state. When going to chart the vaccines after administration vaccine history populated was then that iwas observed that patient was given an extra dose as previous Hepatitis A booster was given back in 2024
