Symptom narrative

Patient was given an extra does of Tdap and Meningococcal ACWY due to system not populating vaccines that were previously given in office. Vaccines were given based off of previous records from out state. When going to chart the vaccines after administration vaccine history populated was then observed that patient was given an extra dose as previous vaccines were given back in 2024
