Symptom narrative

Member got anthrax immunization 3 day ago in RIGHT arm. Member notes since moment received immunization had fevers, chills, body aches. Felt "shaky" on the inside like "drank 30 cups of coffee....off a little bit." -Denies wheezing, SOB, Doe, cough, hemoptysis, rash. -Does have swelling at site of injection, no erythema. Does endorse feeling symptoms down to bone, also w/ difficulty lifting L arm/shoulder abduction. Denies N/T. Has been using lidocaine patch w/o relief. Tried a heating pad last night as well w/o relief. No orals. -Pt hasn't had w/ prior anthrax immunizations, notes this is significantly more painful. Notes currently pain 3/10, TTP, sore. Also notes attendant decreased ROM. -Denies feeling down/depressed. Denies SI/HI. Denies thoughts of losing ctrl and hurting self/others. Denies suicide attempts or preparatory actions ever. Denies passive SI. Denies gambling or excessive risk taking behaviors.
