On 2/9/2026 pt had an appointment with her siblings they both came checked in around 4:04pm to see Dr. their pediatrician both pts I vitals and put them in the room to see Dr. after Dr. did both assessments on the siblings he ask me both siblings needed the flu vaccine so I go to the med room and get them prepared then I enter the exam so I ask who is first to get the flu shots the sister point her finger at her brother to have him go so he said fine I go I'll give him the flu vaccine with no problem. Then I told the pt your up next and she didn't want to get her shot her mom and brother was trying to convince her so as I try to prep and get ready so I can give her the flu vaccine she just kept moving so then I said how about I get the number spray I said to the pt it might hurts less so was opened minded to it so I left the exam room to get the numbing spray then I came back and instructed the pt what I was going do I said to her I need to spay this on first. And she let me proceed then I said I have to give you this flu vaccine or the numbing spray will wear out pt was still not complying with me she kept saying she don t want it so as her mother and brother is trying to sway her to get the flu vaccine I finally got a hold Of the pts arm and after I prep and everything I get ready to inject the flu vaccine intramuscular then as soon as the needle is in and I tried to push the plunger in she jerk her arm away and pt didn't receive her full dose of the flu vaccine. Dr. came in and was giving the mom both Avs for both siblings and Dr. ask if they received both of there flu shots and I explained only the older siblings received there the other one I tried and she jerk her arm around Dr. stated technically she got it even if it was the same dose and we don t repeat it again. So that's how the incident happened
