At 8:13AM (about three minutes after administration of vaccines), client became lightheaded and had a syncopal episode that lasted about 30 seconds before arousing spontaneously. Client had two bouts of dry heaving and stated "I peed myself" while struggling to keep eyes open. Client's lips and skin pale yet moist; she did have urinary incontinence during syncopal episode. Mother concerned, asking if client okay. Provided emotional support to mother, reassuring her that client will be okay. Asked mother if client had anything to eat before coming for her appointment; mother stated client did not have anything to eat this morning. Provided water, teddy graham crackers, cool compress for forehead, and fan. Client lethargic, though responses when name is called and oriented to self and space. RN able to have client sip water and eat Teddy Grahams; client tolerated without becoming nauseated. While observing client, RN educated mother on reaction and interventions client can do before vaccines. After mother calmed and engaged with RN, she stated client's older sibling also had syncopal episode with his last set of vaccines. As client finishes Teddy Grahams, her lip and cheek color returns to pink, and she engages more in the conversation. Alert and orientedx4. Client stated to RN and mother that she remembers feeling the syncopal episode coming, though didn't know how to inform how she was feeling. Reassured client about reaction and provided emotional support. At about 8:35AM, client stated feeling better and asked to stand up; denied any lightheadedness/dizziness. Client changed position to standing; color remained in face and denied any lightheadedness. At 8:40AM, client and mother ambulated with RN from clinic room to front of building (about 80 feet); client tolerated movement. Client left with mother in private vehicle.
