Client states after receiving vaccines, he became sleepy. Writer observed client closing eyes but was easily aroused. Given apple juice to drink and escorted to waiting room for observation following immunizations. Approximately 15 minuets in the waiting room, client fail to the floor from a sitting position. Client assisted to a standing position and walked to the clinical area for assessment. Vital signs taken. (see below). Client refused to go to the emergency department for an evaluation. Immunizations aftercare discussed. Client verbalized understanding of teachings. Client left the clinic in stable condition via ride share.
