Symptom narrative

Client complained of discomfort to the arm while receiving the vaccination. Approximately 5 minutes after receiving the immunization (9:39 am), client stated she wasn't feeling well and became unresponsive to verbal stimuli. She was assisted to a laying position on the floor where her vitals were monitored. Her BP was low (75/31), her legs were elevated and 911 was called. She became arousable after 30-45 seconds. Vitals were stable at 9:44 am with a blood pressure of 115/61 and a pulse rate of 71. Client left with EMS at 9:54 am still "not feeling well" and diaphoretic. She was followed to hospital by her mother.
