Symptom narrative

Patient had titers drawn and was to receive the Hepatitis B vaccination and the MMR vaccination. A nurse had reviewed the chart. The order was placed incorrectly for the meningitis vaccine instead of the Hepatitis B so the wrong vaccine was given by the medical assistant. The medical assistant did administer the correct vaccine per the order, but it was entered incorrectly by the nursing staff. No adverse side effects reported by the patient. He was notified that he still does need to receive the Hepatitis B vaccination. Appointment scheduled.
