Symptom narrative

The member arrived to the clinic on 2/18/26 at 10:48 am scheduled to receive a Hep B vaccine, dose #2. Upon chart review, the member received the 2nd dose of Hep B on 2/5/26 by Clinical Assistant, during an office visit with Dr., which was an invalid dose according to the system as the 1st dose was administered on 1/13/26. According to the system, the 2nd dose was not due until 3/5/26. RN apologized to the member for the misunderstanding and explained the CDC guidelines as to the timeline for receiving the Hep B vaccine. Member verbalized understanding and was rescheduled on 3/5/26 to receive the 2nd dose of Hep B.
