Symptom narrative

Pt received 3 vaccines as part of a pre-hire routine at employee health, one of which was varicella. Had history of 1 varicella vaccine in 2004 needed a second booster. Given Varicella sc in left upper arm. Pt noticed a rash upon waking the next day 02/19 around the site of her varicella vaccine. Called employee health to report same and is currently going for assessment and treatment at the workmed.
