2 month old child received 2 month shots of Vaxelis, PCV 20, and Oral Rotavirus. Intramuscular vaccines received both in left thigh on 2DEC25. Later that day, pt was admitted to ER for swelling in L thigh as well as reported retracted breathing. Mother of patient was informed that swelling may have been due to vaccine, but was unable to know which vaccine due to both vaccines being in same leg. This was report was received verbally to staff 19FEB26 when patient returned to receive 4 month shots. PSR and VAERS report was placed on incident, and mother of patient stated she did feel comfortable proceeding with 4 month shots. 4 month intramuscular vaccines were separated on different legs for precautions.
