Symptom narrative

Patient was supposed to be given PCV 15 at his 15 month appointment on 10/8/25 when without consent he was given the wrong vaccine (capvaxive) by mistake. This error was unfound until another vaccine error occurred on 12/18/25 with the patients sister that prompted a review of the vaccine records. We (the parents) were not notified of the 10/8/25 vaccine error until 1/6/25 almost 3 months later.
