Potential extra dose; This non-serious case was reported by a pharmacist via call center representative and described the occurrence of extra dose administered in a 40-year-old female patient who received Flu Seasonal TIV Dresden (Fluarix 2025-2026 season) (batch number B7C5B, expiry date JUN-2026) for prophylaxis. Previously administered products included Fluarix 2024-2025 season (possible administration of 1st dose of vaccine on 03-OCT-2025). On 12-FEB-2026, the patient received the 2nd dose of Fluarix 2025-2026 season. On 12-FEB-2026, an unknown time after receiving Fluarix 2025-2026 season, the patient experienced extra dose administered (Verbatim: Potential extra dose). The outcome of the extra dose administered was not applicable. This report is made by GSK without prejudice and does not imply any admission or liability for the incident or its consequences. Additional Information: GSK Receipt Date: 13-FEB-2026 The pharmacist reported that was hoping to request any information about patients receiving 2 Fluarix doses in one season (4 months apart). The pharmacist got a potential medication error that they are investigating. The Omnicell showed the flu shot was pulled from the cabinet on 03-OCT-2025 but no documentation that it was given and no documentation that the dose was returned to the Omnicell either. The patient just received a dose (per the documentation) last week, so they are not sure if they definitely got 2 doses but wondering what the consequences/safety implications are if it did reach the patient twice in one season.
