Patient, a 4-year-old female, was brought in for an acute visit due to swelling and localized reaction at the right leg injection site following her four-year-old vaccines. Her caregiver described redness, soreness, tightness, and warmth developing over the weekend, but Patient did not have fever or significant pain. The caregiver stated ER provider was concerned about the number of vaccines combined and the risk of reactions, noting this was Patient's first such experience. Patient received oral steroid and Benadryl in the emergency room. She remained active after medication and managed symptoms with cold packs. She stayed home from school and had a note for her absence. Swelling has greatly improved since yesterday and remains afebrile. Suspect likely resolving localized reaction, cellulitis less likely - Ordered cetirizine liquid, once daily, to help with redness and swelling. - Continue Tylenol every 4-6 hours as needed for pain or swelling. - Continue cold pack at home. - Monitor for fever or worsening swelling. - Plan to space out future vaccines.
